INDEMNITY:

I, as the Parent/ Legal Guardian hereby apply for membership of the Milnerton Cricket Club on behalf of the Junior Member applicant as stipulated in the "Milnerton Cricket Club Junior Registration Form 2026" (hereafter referred to as THE APPLICATION FORM) for the period September 2026 to April 2027.

I agree to abide by the WPCA By Laws for 2025-2026; WPCA Code of Behaviour 2025-26; Milnerton Cricket Club: Code of Conduct; Milnerton Cricket Club Constitution and agree to pay the applicable membership subscription fees levied by the club for the above period on application. I understand that that completion of this application does not guarantee my child's acceptance into Milnerton Cricket Club Junior Section.

I, the Parent/s/ Legal Guardian/s (hereafter referred to as GUARDIAN) of my child (hereafter referred to as MEMBER) residing at the address stated in THE APPLICATION FORM, do herby irrevocably indemnify Milnerton CRICKET CLUB (hereafter referred to as THE CLUB)and the agents/ officials/ sponsors of THE CLUB, against any claims for delictual damages by, or on behalf of, said MEMBER, as direct or indirect consequences of his/her participating in practice sessions, official, competition or friendly match fixtures which THE CLUB may organize, or party to the organization of, properly constituted under the jurisdiction of Western Province Cricket Association(WPCA), the relevant cricket authority.

Furthermore, I, the GUARDIAN of the MEMBER hereby freely give my/ our consent that the MEMBER is permitted to participate in said sports fixtures and practice sessions, for THE CLUB, with my/our full knowledge and appreciation of the inherent risks associated to playing and participating in cricket activities, and indemnify THE CLUB, its agents and officials against any liability for any claims that may arise there from. I/We indemnify THE CLUB, its agents, officials and parents against liability against any claims for delictual liability which may arise from the transporting of said MEMBER to and from any place where fixtures is played, as well as while said MEMBER is at any place where the sports activity/ fixture is played whether supervised or unsupervised.

By undersigning this application, the signatory confirms that:

1. I acknowledge that the use of the facilities offered by the Milnerton Cricket Club shall be at my own risk, and I hereby indemnify the Milnerton Cricket Club against all loss, damage, injuries, costs, charges and expenses which I may sustain or incur arising out of, or attributable to my utilizing these facilities.

2. I agree to pay the annual membership fee (as above) before or on 30 September 2026. Total Annual Subs R2300

3. I undertake to in writing request for the down payment before 30th September 2026 of my annual membership fee should I not be able to pay it in full before the due date. Failure to make down payment as agreed will result in the annual subscription fee becoming payable immediately.

4. Members not paying their annual membership fee fully on or before 30 September 2026 will not be eligible for selection for any team until payment of the annual membership fee is received in full.

6. I understand that I remain responsible for paying the Cricket Membership Fees in full, regardless of any circumstances arising due to injury or any other matter which prevents me from completing the 2026/27 season.

7. The only method of payment is Electronic Fund Transfer. (use your initial, surname and "subs" as EFT reference).

8. I agree to be bound by the Selection Policy established by the Milnerton Cricket Club from time to time.

9. By signing this membership form, I agree to the terms and conditions thereof.
